St. Brendan utilizes the company Smart Tuition for tuition payments. Learn more at www.smarttuition.com

Please return all of the necessary forms for Registration:

1. Complete Registration Application
2. New families must submit complete student records including:
   • Copies of transcripts from previous schools
   • Baptismal certificate
   • Birth certificate
   • Social security number
   • Immunization forms/Health records (State of Florida)
3. Select method of Tuition payment:
   • Monthly – Complete automatic payment withdrawal form (Smart Tuition);(Note: All fees may also be included in your Smart Tuition payment)
   • Semi-annually – Two payments: 1st payment date August 1; 2nd payment date January 1
   • Annually – One payment due August 1
4. Complete tuition aid forms if applicable.

PLEASE NOTE:

• Only complete registration packages will be processed
• Registration forms will not be accepted for accounts with past due balances
• All fees are non - refundable
