Please return all of the necessary forms for Registration:
- Complete Registration Application
- New families must submit complete student records including:
- Copies of transcripts from previous schools
- Baptismal certificate
- Birth certificate
- Social security number
- Immunization forms/Health records (State of Florida)
- Select method of Tuition payment:
- Monthly – Complete automatic payment withdrawal form (Smart Tuition);(Note: All fees may also be included in your Smart Tuition payment)
- Semi-annually – Two payments: 1st payment date August 1; 2nd payment date January 1
- Annually – One payment due August 1
- Complete tuition aid forms if applicable.
PLEASE NOTE:
- Only complete registration packages will be processed
- Registration forms will not be accepted for accounts with past due balances
- All fees are non - refundable
